I have my NST set to block zip file attachments. I have tested this setting as well by sending a zip file. I had an issue where an attachment came in from an email address in a domain I have white-listed; and it did not block the attachment. I want to know why the attachment was not blocked on this email and how can I correct this issue.

This sounds like a configuration error.
Please your configuration files directly to the NST Support Team for inspection.
