I have this line in my local.cf with an email address following. The mails sent to this address appear to be rejected but I can't see that happening in the log.

Should the log show recipient rejected in some way? All I see in the log is that the mail appears to be rejected as SPAM as any other SPAM message.

BLACKLIST_TO would appear like any other rule in the SpamAssassin report, if you get it. If you choose to outright delete spam mails, you'll never know what happens exactly, unless you enable "log filter errors" and turn on the debug mode of SpamAssassin (by adding -D to the command line). But this gets you loads of information in the log.

I recommend using reject/deliver for spam mails, and then look at the report if everything is as expected.
